Testimonial and Photo Release: In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to UB PAINTN & SIPN LLC, organizers and representatives, the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations. The rights granted herein shall extend in perpetuity, unless revoked in writing to the UB PAINTN & SIPN LLC by me, throughout the world and for any purpose whatsoever, including without limitation for marketing and advertising purposes of the UB PAINTN & SIPN LLC and its agents, and in any and all media, including without limitation the UB PAINTN & SIPN'S website and social media outlets. I acknowledge that the UB PAINTN & SIPN LLC and its agents have no obligation to return any photographs or images to me. I hereby RELEASE, WAIVE and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use by the UB PAINTN & SIPN LLC and its agents, including without limitation any and all claims for libel or invasion of privacy. I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name (if under 18, I have the right to represent all children under 18 during our visit to the UB PAINTN & SIPN LLC and waive all/any obligations to the UB PAINTN & SIPN LLC. I have read the above Release and am fully familiar with the contents thereof. This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.

I have read this Testimonial and Release Form, fully understanding its terms, that I give up substantial rights by signing (printing my name) and sign it voluntarily.

Personal Responsibility with Alcohol: Consumption of any alcohol at events is optional and a personal decision. By running with (UB PAINTN & SIPN LLC, I agree that any alcoholic beverages I consume are my own choice and responsibility. I agree to drink responsibility and heed suggestions from others and will not drive if I am intoxicated. I acknowledge that I alone am responsible for all of my actions resulting from consuming alcohol and the  UB PAINTN & SIPN LLC is in no way responsible.

 

Waiver: I agree that I am a customer of the, UB PAINTN & SIPN LLC and I know that upon entering in the UB PAINTN & SIPN LLC and participating in any UB PAINTN & SIPN LLCactivities, In-studio activities and off-studio events!

I attest that I have read and agree to abide by the rules of the.  UB PAINTN & SIPN LLC I assume all risks associated with being a customer of this UB PAINTN & SIPN LLC and participating in UB PAINTN & SIPN LLC activities which may includes: PAINTING, WINE TASTING, the usage of art supplies (such as scissors, staplers or any of arts and crafts tools, along with any legal consumption of alcoholic beverages (Adults 21 and older); and contact with other participants during COVID or any PANDEMIC EMERGENCIES! We are not responsible for any medical illnesses due to contact within the UB PAINTN & SIPN .

Having read this waiver and knowing these facts and in consideration of you being a patron of the UB PAINTN & SIPN, I, for myself and anyone entitled to act on my behalf, waive and release this UB PAINTN & SIPN LLC, the UB PAINTN & SIPN LLC organizers,  and all UB PAINTN & SIPN LLC sponsors, their agents, representatives and successors from all claims or liabilities of any kind arising out of my participation with the UB PAINTN & SIPN , even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver.
